A Long Term Condition (LTC) review is a yearly assessment for patients with a chronic condition(s), to ensure condition(s) are being managed effectively.
It is important to have your review, in order for the GP to continue prescribing any medication you receive.
The review may involve different elements, such as blood tests, physical examinations, and lifestyle questions.
Patients will be invited to attend for review in their Birthday month, during the LTC review year (April to March). Patients who attend for review outside of their Birthday month will still be called on their Birthday month the following LTC year, resulting in the next review being less/more than a year.
