We are changing how we organise chronic disease reviews.
From now on, please book all your chronic disease reviews in the month of your birthday.
While we change to this new system the chronic disease reviews this year may be earlier or later than expected.
If you have already had your annual review for a chronic condition this year (since 1st April) you will not need to have another, however if your review was before then you will need to book one in the month of your birthday.
We will contact you, usually by text / email, to remind you to book a review appointment. If you can please fill in the questionnaire sent with the reminder message. If you are able to complete it, the information is saved directly into your record, allowing the nurse to view it before you arrive. This will then give more time for your review.
Don't worry if your phone doesn't allow you to complete this, we can gather the information directly at your appointment.
